At Jach Family Wellness Center we want to give you options. We offer different ways to pay so that you can choose that option that works best for your life. Give us a call at (708) 957-1400 if you have any questions.

• We are in-network with Blue Cross & Blue Shield of Illinois
• We accept Medicare
• We accept most other insurances. Give us a call to confirm your insurance.
• We offer a self-pay discount program that gives patients that want to self-pay the same rates as patients who want to use insurance.
• We do not accept Medicaid at this time.
